Patient Info

Your initial evaluation will be a one-hour visit with a licensed physical therapist. We will take time to learn about your medical history and the details about your condition. Please bring copies of any medical reports (MRI, CT or surgical report). We will conduct a thorough physical examination and develop a detailed treatment plan. Often, we begin your treatment during your first visit and will provide you with a home exercise program. Please wear loose fitting clothing to your appointment. If you are coming before or after work, we have rooms where you can change.

In order to save time during your first appointment, we encourage you to download and complete the new patient paperwork in advance. If you are unable to complete your paperwork at home, please arrive 30 minutes before your appointment time to complete your paperwork.

UM Charles Regional Rehabilitation accepts many insurance plans, including most managed care plans. If you have questions regarding whether or not an insurance plan is accepted, please call 301.609.5494. The following is a general list of accepted insurances:

When you call to schedule your first appointment, we will contact your insurance company to verify that it covers physical therapy. Per our contracts with insurance companies, we are required to collect co-pays at time of service. You will receive an invoice for co-insurances and deductibles.
